Please note: A separate registration form must be completed for each attendee.

Cancellation Policy: Written notice of cancellation must be received no later than April 30, in order to receive a refund, less $75 processing fee. There will be no refunds for cancellations received after this date. Substitutions can be made at any time in writing. No shows will be charged at the full conference rate.
